## Regional center intake and service coordination

Up to age 17

Harbor Regional Center is the California regional center for its service area. Families apply online; the center assesses eligibility and, once a child is eligible, assigns a service coordinator. Infants and toddlers under 3 get an Individual Family Service Plan (Early Start); children 3 and older get an Individual Program Plan through person-centered planning meetings with the family. Most services and supports are free regardless of age or income once eligibility is determined, with a few exceptions, and the center cannot pay for services another agency, such as schools or health insurance, is responsible for. Separate intake lines serve children under 3 and those 3 and over. Applicants must live in Harbor's service area. The center also offers a Self-Determination Program.

## About Harbor Regional Center

Harbor Regional Center is one of California's regional centers. It serves people with intellectual and developmental disabilities and their families, including infants and toddlers with developmental delays or at risk of a developmental disability. It determines eligibility, then assigns a service coordinator who plans, arranges and monitors services and supports. It also runs Family Resource Centers and a Self-Determination Program, and works with a community of service providers.
