Families apply and are assessed for eligibility; people found eligible are assigned a Service Coordinator who meets with the family, learns their goals and builds a plan of support naming the services needed and who will provide them. For clients over age three this is the Individual Program Plan. The site has sections for early intervention, school-aged children, adulthood and families, plus resources on behavioral health treatment, assistive technology, dental information and special education.
